It is responsibility of CGHS card holder to keep the card safely in his custody. If in case CGHS plastic card is lost or damaged the card holder has to immediately report to AD Office, New Delhi and department in which the member is working. When the department will receive complete details regarding misplacement of card it will send the detailed report to Additional Director Office.
If the card is lost, it can be misused. So an immediate FIR should be launched to stop misuse of CGHS card. The member has to report to Additional Director (HQ), New Delhi along with copy of FIR. The member should also inform CMO In-Charge of the wellness center or hospital where your CGHS card is registered. To get duplicate card the CGHS member has to pay a penalty of Rs.50/-. Penalty once paid is not refundable even if the card is retrieved later on.
Following points are to be kept in mind regarding Loss of CGHS Plastic Card
- If duplicate CGHS plastic card is issued and later on the old card is found, then it should be reported to AD Office and the original card should be returned for cancellation.
- In special circumstances the Health and Family welfare ministry may waive off the penalty for loss of card.
- If the Card holder fails to provide any satisfactorily reason for loss of card, the authorities may take disciplinary action against him.
CGHS Plastic Card
- CGHS card is not transferable.
- IF anyone misuses CGHS card penalty may be imposed and if it is misused by any Government employee, disciplinary action may be taken against him/her.
- After expiry of CGHS card it has to be surrendered at the concerned department. If CGHS card renewal is required then required form is to be filled and submitted. - The CGHS card holder must surrender the card to the concerned department if he/she is shifting to a non CGHS covered area.
CGHS Plastic Card Colour Scheme
CGHS card of white color having a colored strip at top. These colored strips are of four colors namely Red, Blue, Green and Yellow. This colored strip shows the holder status of card.
- Red color striped card – Members of parliament are issued cards with red color strip at top.
- Blue color striped card – Blue striped CGHS plastic card shows that the card belong to Serving Government Employees.
- Green color striped card – Pensioners, Retired Personnel, Freedom Fighters and ex-MPs (Member of parliament)
- Yellow color striped card – Journalists and Beneficiaries of Autonomous Bodies.
Sir I work in Delhi and I fall ill and went to Dehradun during medical leave for better care as water air are contiminated in Delhi and there is no one in Delhi to care. In Dehradun my plastic CGHS card lost and missing report was lodged in Chowki Patelnagar Dehradun. I returned to Delhi on 14 Mar 17 and now I want to get CGHS facility while I have lost my CGHS card and I am currently under medical leave and cannot join office without fitness certificate. what should I do, pl help.

Don’t worry if you lost your CGHS card. Just follow these instructions:
1. Register your FIR and obtain the receipt.
2. Purchase a postal order for Rs 50/- in the name of ‘PAY AND ACCOUNT OFFICER, CGHS’
3.Get ready an application to the additional registrar, 2 photographs, copy of Adhar card and a photocopy of lost CGHS card if available.
Now contact CMO of your dispensary and submit your application attaching all the above documents
Following documents are required for duplicate CGHS Card:
1. Application
2. FIR
3. Postal Order of Rs 50/- in the name of ‘ PAY & A/C officer, CGHS’
4. Photocopy of Adhar Card
5. Photocopy of lost CGHS Card

I am working in Chennai(Monitoring Directorate, Central Water Commission, West Velachery-60042) and I fell ill during July 2019 while I was going to my Coimbatore residence.Then I was under medical treatment from 18.07.19 onward . I lost my shoulder bag containing my spectacles, cloths, Key of my house and everything. I have lost my credit / debit card and money from my pocket . My CGHS card of mine and my elder son also lost which was in the shoulder bag. The copy of FIR filed on line is sent herewith . I request you to issue a new CGHS card to me and my elder son at the earliest please.

I’m a former member of Parliament in the house of Rajya Sabha. Therefore, I am entitled to CGHS card. But unfortunately a mistake has been done in the spelling of my surname while printing the card. Accordingly, the wrong spelling of my surname has been printed on my card.

I’m retired central govt employee and retired from revenue department as Asstt. Commr. in the month of February 2018. Thereafter I applied for CGHS cards for pensioners in May 2018 and the beneficiary id is 3864358 for myself, 3864359 on behalf of my wife Mrs. Chandrima Saha and 3864360 on behalf of my daughter Ms. Soumyapriya Saha.
Till date I have not any plastic card from my deginated CGHS dispensary no. 3 ( Dover Lane, Kol 3) after the lapse of more than 2 years. Please take the ineciative action to issue the said card.
