Eligibility for CGHS Card are for following:
- Central Govt. Retired employees and dependent family members receiving pension from Central Civil Estimates
- Current members of Parliament and Ex-Members
- Lt. Governors and Ex-Governors
- Freedom Fighters & Ex-Vice Presidents
- li>Current and Ex-Judges of High Courts & Supreme Court
- Pensioners and Employees of some specific autonomous organizations in New Delhi,
- Railway employees
- Delhi Journalists having PIB (for medical facilities & OPD at Dr. RML Hospital, New Delhi
- Delhi Police Personnel appointed in Delhi only
- Central Government Servants who got absorbed through proper channel in an Autonomous or Statutory Bodies or Central Area Undertakings and are in receiving of pension by Central Civil Estimates.
Registration procedure and Documentation for CGHS Cards
- Pensioners:
- CGHS Cards can be issued from the JD / AD office of the City.
- You can download forms from CGHS Website or can take forms from the AD / JD office.
- Supporting Documents Required:
- Application in appropriate forma
- Residence ID proof
- Proof of living of dependents
- Age proof of a son
- Disability certificate (if the age of the son is 25 or above, otherwise he is not entitled as beneficiary)
- Recent Photos of dependent family members
- During the service period, if you had any CGHS Card, then Surrender Certificate of previous CGHS Card is required
- Attested PPO copied & Last Certificate of payment
- Demand Draft of required amount towards CGHS contribution is to be issued for ‘AD, CGHS of the city’ and ‘P.A.O., CGHS New Delhi’ in Delhi.
- If PPO is not ready for any particular reason then there is another option to get a provisional card as per Last Payment Certificate.
- The all above data will be scanned and recorded in a database through computers and a printout will be issued same day for temporary use. Through registered post, Plastic cards will be sent to the residence address of the cardholder.
- For Serving employees: Attach the photos of eligible family members and Submit the filled forms in the required format to the Office or Department of Ministry. The submitted application form will be further forwarded to the office of Addl. Director CGHS of city to issue of CGHS Card. Print out can be issued for immediate use.
For serving employees checking of eligibility for CGHS card is easy because it is compulsory for them. The Deduction from salary is made by the department, every month, depending upon their pay w.e.f 1st June 2009 Grade Pay w.e.f. 1st June 2009.

If the treatment places are not covered by the CGHS, then how the expenses amount will be reimbursed?
If any Central Govt the employee or his family members covered under CGHS suffers from illness at a place that is not covered under CGHS the treatment shall be treated under CS (MA) Rules. The reimbursement of medical expenses in such cases shall be compared to the amount spent for treatment in a Govt. / State Govt hospital in that city.
When CGHS subscribed employees are transferred to NON-CGHS areas, their families continue to stay back at the old duty station. Here, they will be able to get CGHS medical facilities which can protect them from additional expenses and other financial issues. So the eligibility for CGHS cards are profitable for those people who stays inside the CGHS covered areas.
As per Central Government Health Scheme Rules, in CGHS cards, who can be mentioned as dependent or members of “family”? The dependent on the family would be a wife or Husband and other Dependent Family Members such as widowed sisters, parents, sisters, minor brothers, widowed daughters and minor sister, stepmother, separated or divorced daughters with their minor children, stepchildren and children and wholly dependent upon the Govt. Employees and are normally staying with the Govt employees.
The dependents can be said as the person whose total income from all sources, including pensions and an amount equal to DCRG benefit is less than Rs.3500/ and earned DA on Basic pension amount of Rs. 3500/ per month. But there is no such clause mentioned for spouse.
Can the dependent in-laws be eligible for CGHS Facilities?
A lady Govt. Servant can choose either her own parents–in-law or parents as dependents for getting the benefits under CGHS subject to the terms and condition of dependency and residence, etc. But this option is invalid for male Government servants.
Case study:
Query: My daughter is divorced from her husband and now doing job in private sector. Her total monthly salary is INR 10,000/-. Is she eligible for getting CGHS facilities on behalf of me as I am a retired Governor?
Answer: No. As she is able to earn a reasonable amount and she can be eligible to get ESIS facilities. So, she is not eligible to get CGHS facilities as your dependent.
