How to register with CGHS

Employees and Pensioners of Central Government can register with CGHS online. One can apply for CGHS plastic card by filling form online and submitting it.

For Pensioners

You have to fill form online then print it and get it verified from the authorized person.

After verification attach the photographs and required documents and submit the application form at the office of Additional Director or Joint Director (AD/JD) CGHS.

For Serving Employees

In the same way serving employees have to fill the form enclose the photographs and necessary documents and submit the form at the concerned office/department where he/she is employed. Then the department forwards the form to Additional Director of CGHS for issue of CGHS plastic card. The printout of form can be used temporarily to avail facilities at CGHS wellness center.

If Pension Pay Order is not ready then one can get a provisional card on basis of last pay certificate. After all the data is entered, a printout is provided for immediate use and later on the plastic cards are sent by post to card holder.

Central Government Employees residing in CGHS covered cities have to compulsorily get CGHS card. For serving employees a fixed deduction is done from their salary every month

How to calculate Draft amount for CGHS card for pensioners

For Retired Central Government Employees who want to avail CGHS benefits can make contribution in two ways.

  1. They can pay on yearly basis
  2. They can pay one time (10 years contribution) and avail CGHS benefits for lifetime.

The contribution by pensioners is calculated on the basis of their Grade pay when they were in service.

FAQ for CGHS Plastic Card

When I will get my CGHS card?

A computerized index card is issued immediately and in normal routine CGHS Plastic cards are sent to CGHS beneficiary at postal address in 3 weeks.

If I do not get card in 3-4 weeks whom I should contact?

If you do not get card in prescribed time you should contact Joint Director, Head Quarter, CGHS, Bikaner House, New Delhi (for Delhi) and Additional Director of City (for other cities)

I have lost my CGHS card what I should now?

The beneficiary is solely responsible for CGHS card. In case if you have lost your card, you have to immediately report for loss of card to Additional Director (HQ), CGHS, New Delhi and also to your department. You have to apply for duplicate card to Office of the Additional Director (HQ), CGHS, New Delhi providing all the details regarding loss of card and  penalty of Rs.50/- . To pay penalty you have to submit Postal Order of Rs.50/- payable “PAO, CGHS New Delhi” for Delhi and “AD, CGHS city” for other cities.
