How to get a New Pensioners CGHS Card

If you are a pensioner and retired from central government job you can avail CGHS facilities by applying for CGHS card. The card can be applied at Assistant Director (AD) or Joint Director (JD) office of the city. One can download the form to apply for Pensioners CGHS Card here or can physically collect form from AD or JD office.

While applying for the card one has to submit the following proofs or documents.

  1. Residence Proof
  2. Dependents Stay Proof
  3. Proof of age of son if adding him as beneficiary
  4. If age of son is more than 25 years certificate of disability to add him as beneficiary
  5. Surrender certificate of CGHS certificate while in service
  6. Last Pay Certificate and attested copy of PPO
  7. Draft for the amount to be submitted for 10 years contribution towards CGHS
  8. The DD should be in favor of “P.A.O., CGHS New Delhi’  in Delhi and  in the name of “Additional Director, CGHS of the city”
  9. If you do not have PPO ready, you can get provisional card valid for 6 months on basis of last Pay Certificate.

Facilities provided to CGHS Beneficiaries are not free of cost. On has to pay contribution towards it. There are 2 options to pay for it. One you can pay on yearly basis. Second you can pay for lifetime membership by paying 10 years contribution.

Endorsement of entitlement of facility at hospital (General/Semi-Private/Private Ward) depends on Basic Pay/ Pension. You have to get your CGHS card registered at your nearest Wellness center to avail benefits. Once you get your card registered at your nearest Wellness center and later on shifts to another place it is your willingness if you want to shift to another wellness center or not. To shift your CGHS card from one wellness center to another, you have to apply in name of CMO In charge of Wellness center.

If you are retired from CGHS and shifted to a non-covered CGHS city, you can avail benefits at the nearest CGHS covered city.
