It is responsibility of CGHS card holder to keep the card safely in his custody. If in case CGHS plastic card is lost or damaged the card holder has to immediately report to AD Office, New Delhi and department in which the member is working. When the department will receive complete details regarding misplacement of card it will send the detailed report to Additional Director Office.
If the card is lost, it can be misused. So an immediate FIR should be launched to stop misuse of CGHS card. The member has to report to Additional Director (HQ), New Delhi along with copy of FIR. The member should also inform CMO In-Charge of the wellness center or hospital where your CGHS card is registered. To get duplicate card the CGHS member has to pay a penalty of Rs.50/-. Penalty once paid is not refundable even if the card is retrieved later on.
Following points are to be kept in mind regarding Loss of CGHS Plastic Card
- If duplicate CGHS plastic card is issued and later on the old card is found, then it should be reported to AD Office and the original card should be returned for cancellation.
- In special circumstances the Health and Family welfare ministry may waive off the penalty for loss of card.
- If the Card holder fails to provide any satisfactorily reason for loss of card, the authorities may take disciplinary action against him.
CGHS Plastic Card
- CGHS card is not transferable.
- IF anyone misuses CGHS card penalty may be imposed and if it is misused by any Government employee, disciplinary action may be taken against him/her.
- After expiry of CGHS card it has to be surrendered at the concerned department. If CGHS card renewal is required then required form is to be filled and submitted. - The CGHS card holder must surrender the card to the concerned department if he/she is shifting to a non CGHS covered area.
CGHS Plastic Card Colour Scheme
CGHS card of white color having a colored strip at top. These colored strips are of four colors namely Red, Blue, Green and Yellow. This colored strip shows the holder status of card.
- Red color striped card – Members of parliament are issued cards with red color strip at top.
- Blue color striped card – Blue striped CGHS plastic card shows that the card belong to Serving Government Employees.
- Green color striped card – Pensioners, Retired Personnel, Freedom Fighters and ex-MPs (Member of parliament)
- Yellow color striped card – Journalists and Beneficiaries of Autonomous Bodies.
